Health Services for Agnews Consumers in the Community

The below documents provide information on health services for Agnews consumers who have transitioned to the community. The documents include policies and procedures that guide how the Department of Health Care Services (DHCS), DDS, regional centers, and designated health plans provide services and supports in the community. These procedures and policies are included in a Memorandum of Understanding (MOU) between DHCS and DDS, as well as regional centers and health plans. In addition, the "Health Services for Agnews Consumers in the Community: An Overview" has been developed to explain how health care and supportive services will be provided in the community.
